Medical questionnaires are available at various moments of the patient care pathway in the “Surveys” section.
From the moment a questionnaire becomes available, it appears in the section and can be completed by the patient.
Start a survey
To begin answering a questionnaire, click on “Start” button in the section.
The screen will then provide an overview of the questionnaire content. Click “Start” again to continue.
Fill out a survey
The patient can now complete the questionnaire question by question.
It is possible to move from one question to the next by scrolling the screen from top to bottom or by clicking on “Next question“.
At the end of the questionnaire, there is the “send” button. Once all the questions have been answered, click on the button to submit the responses.
Once a questionnaire is started, it’s not possible to go back to the app until it’s completed.
Submitting a survey
A confirmation of submission is displayed on the screen.
This means that the data is recorded, and the care team can access it. The questionnaire disappears from the patient’s “Surveys” section.
To return to the overview, click the “Go back to the overview” button at the bottom of the screen.
