
1. Goal of phase 2
In this second phase, we will focus on:
- Which preventive actions you can take to avoid a COPD attack
- Which actions you can take when you have signs of a possible COPD attack
2. What will happen during phase 2?
a) COPD action plan and (self-)monitoring
Instead of a daily questionnaire, you will receive the COPD action plan. The COPD action plan is a single question about the type of day you are having: a green (good) day, a yellow (bad) day or a red (very bad) day. Based on your answer, you will receive suggestions of actions you can take to improve how you are feeling.
If your parameters are stable and you feel well, you will no longer be monitored on a daily but on a weekly basis by your medical team. If you are having a bad day, you can choose to fill in the daily questionnaire. This way your medical team can still monitor you on bad days and give advice when needed.

b) Coaching & activity advice
In the first phase of your digital care journey, you learned which parameters and signs are important to recognize to prevent a new COPD attack. In this second phase we will focus on which (preventive) actions you can take to avoid one.
One of the actions you can take is to learn how to pace your daily activities and how to find a healthy balance between being active and resting. Your medical care team will coach you by providing personalized exercises and advice on your daily activity levels with the help of your activity tracker.
c) Education
Also, in this phase, you will receive some information modules. In these modules, you will learn some principles of self-management, preventive actions you can take to avoid a new COPD attack, the impact of the weather, etc.

3. Going to phase 3
If you meet the following criteria you can move on to phase 3:
- You know the core components of self-management to prevent an exacerbation
- You understand and know what you can do when you show signs of a potential exacerbation
