Telerehabilitation convention nomenclature codes

Thecodes to be used in the convention are mentioned in the “Agreement between the Insurance Committee of the Medical care service of Riziv, the physiotherapists and accredited nursing establishments for the reimbursement of the rehabilitation of patients before and after primary knee or hip arthroplasty with hip arthroplasty with a mobile application”.

This agreement is signed by the hospital and is available in Dutch and French on the RIZIV website.

The forfaits cannot be combined, during the same period, with codes provided for in Articles 7 and 22 of the nomenclature or with the tele-benefits provided for in Article 13 of Royal Decree No 20 of 13 May 2020 for rehabilitation following primary knee or hip arthroplasty.

The physiotherapist may not add any supplements to these forfaits during the entire pre- and postoperative care process.

Forfait 1: Preoperative phase

Forfait 2: Mobile application

This forfait is meant for moveUP (not a medical act, but forfait for the app). It has to be attested together with F1 and F3 (the medical act). moveUP will invoice the physiotherapist of F1 or F3.

The digital monitoring during the hospital stay does not interfere with the treatment of the treatment of the patient in the hospital, but merely involves the digital follow-up of the  the data entered by the patient. 

This digital follow-up, provided by the mobile application, contains the following components:

  1. General
    • Manual and technical support for the use of the mobile application and platform;
    • Improvement and updating of the mobile application and platform, so that the safety and security and confidentiality of the data (transfer) remain guaranteed;
    • Informed consent of patient and healthcare provider;
    • Long-term follow-up (i.e. 3, 6 and 12 months after the operation).
  2. With regard to the healthcare provider
    • Training in the use of and access to the platform;
    • Preoperative digital evaluation to support physiotherapist and orthopaedic surgical specialist
    • Platform to provide the patient with a daily rehabilitation programme (with a combination of activities, medication and exercises) updated through the processing and analysis of objective data (e.g. via wearable measurements) and subjective data (e.g. via short questionnaire)
    • Alert function via the platform for the treating physiotherapist for referral of the patient to his surgeon or general practitioner
    • Evidence of activities (report)
  3. With regard to the patient
    • Rehabilitation accessories: mobile application and actvitiy tracker (+ mobile device for rent if necessary)
    • Preoperative education via the mobile application; 
    • Individualised treatment tailored to the patient via the mobile application, including activity (level) advice, exercises, monitoring of medication use and coaching via active monitoring of the processed and analysed data by the physiotherapist in charge. 
    • Medium for data exchange between patient and care provider via structural questionnaires, validated measurements (including range of motion, gait analysis) and the communication system. 
    • Alert function via the platform to the patient for referral to the treating physiotherapist or his GP; 

Forfait 3: Digital follow-up by physiotherapist

Forfait 3 is for the follow-up, (tele)monitoring and adjustment of the therapy. It includes:

  • all the services provided via the system as well as all services related to the rehabilitation carried out by the physiotherapist during the entire postoperative care process
  • the final report
  • This also includes the classical sessions carried out with the physical presence of the physiotherapist (obliged or on indication)

Knee arthroplasty

If the patient stops therapy before the end of the 12 weeks period for knee arthroplasty, the reason for stopping must be recorded. 

Arm 2: A minimum number of classical sessions with physical presence of the physiotherapist per week is required

Hip arthroplasty

If the patient stops therapy before the end of the 8 weeks period for hip arthroplasty, the reason for stopping must be recorded. If the treatment takes longer than 8 weeks this should be motivated by a specialist doctor in orthopaedic surgery or physical medicine and rehabilitation
